ISTA Training Student Indemnity Agreement

PLEASE READ CAREFULLY BEFORE SIGNING.

 

I, _______________________________________ the undersigned, agree to the following:

 

1. That I will fully participate in this workshop in which I am registered.  I understand that I am participating at my own risk.

 

2. That I am physically able to do such workshop or program and that I have no medical problems which may be affected by these activities and any other activity at the workshop /program, such as dance, movement, breath, energy work, etc.  I agree to monitor my participation and not exceed my own limits and I assume all risk for my condition in this workshop.  I acknowledge that I assume all risk from any consequences due to any pre-existing

physical or psychological conditions that I have.  If I become aware of such a condition or problem during the workshop / program I agree to contact my physician immediately.

 

3. That I will refrain from consuming any form of recreational and/or illegal drugs while in attendance at the workshop / program.  Prescribed medications are permitted at participant’s own risk.

 

4. That in lawful consideration for participating in this workshop / program, I hereby agree that I, my heirs, assigns, guardians, distributes, personal representatives, executors, spouse and relatives will not make a claim against, or file a lawsuit, attach the property of, or prosecute the Organizer or Facilitators or their sponsors, agents, co-teachers, employees, partners, assigns, and successors in interest, for injury or damage resulting from participation in this workshop / program as listed below, or other acts, however caused, by any employee, agent, sponsor, co-teacher, volunteer, or contractor of the Organizer or Facilitators, or as a result of my participation in this workshop / program in the following ways.

 

(a) attendance at this workshop/program;

(b) all physical contact with other persons at the workshop / program;

(c) all other recreational activities at the workshop / program;

(d) automobile travel to, or from, or during any activity;

(e) any other activity at the workshop / program.

 

5. Further, in consideration of the presentation of the activity, I agree to release, and forever discharge the Organizer and Facilitators and their sponsors, agents, co-teachers, employees, owner, successors and assigns from any, and all such liability, responsibility, and obligation to them arising out this workshop / program.

 

6. I further agree to indemnify the Organizer and Facilitators and their sponsors, agents, co-teachers, employees, owner, successors and assigns and hold them harmless, from any and all liability, responsibility and obligation to them, including but not limited to liability for my participation in this workshop / program, and acts which may be committed in this workshop / program as listed above, and which may result in injuries to me.  This agreement to

indemnify and hold harmless extends to my heirs, assigns, guardians, personal representatives, executors and relatives.  This indemnity agreement extends to all costs of litigation, including attorneys’ fees.
